Welcome To "Our Final Journey"
 When a loved one is facing a terminal illness, or end of life, families are naturally devastated and often don't know where to turn for advice. There are many issues such as a Health Care Power of Attorney, Living Will, DNR, Last Wishes, Caregiving needs, Hospice, etc. that all need to be considered.

Discussing these issues has always been very difficult for most people to do, until the time comes when they are faced with a situation where they need immediate help. Having been put in the situation of needing this support when both my parents died and in caring for my daughter Kelly throughout her 15 year battle with Huntington's Disease, I found out the hard way how much better this path in life can be when you make most of the difficult
decisions at a time well before the need arises.

I hope this directory of links I've found useful will be of help to you in your time of need. There are several excellent checklists, forms and information to help you in planning for your family needs whether you have a chronic medical condition or not.
