Community Services Program
The purpose of the Community Services Program is to provide support services to older adults and persons with physical disabilities so they can remain in their home. Services include: Case Management, Transportation, Health Maintenance, Information and Assistance, Outreach, Senior Companion, Congregate Meals, and Home Delivered Meals.
 
Personal Cares Program
The purpose of the Personal Care Program is to provide individualized care to older adults and persons with physical disabilities so they can remain in the least restrictive environment. Services include: Respite Care, Personal Care, Adult Family Foster Care, Family Home Care, Adult Day Care, Training for Family Caregivers, Traumatic Brain Injury Services-Residential and Transitional Living, Adult Residential Services.
 
Home Safety Program
The purpose of the Home Safety Program is to provide individual interventions, service equipment to older adults and persons with physical disabilities so they can feel safe and independent in their home environment. Services include: Emergency Response System, Home Injury Prevention, Chore, Homemaker, Environmental Modifications, Specialized Equipment and Supplies.
